site is owned and made available by Massachusetts Consumer
Health (hereinafter “MACH”), which reserves
the right to change, add, modify or remove any part
of our terms and conditions at any time.
NOTE: If you visit or submit information to this site, you accept these conditions. Therefore, please read these terms and conditions carefully.
Purpose of the site: The purpose of this site is to help Massachusetts health insurance consumers obtain insurance quotes from participating MACH agents, brokers and carriers, all of whom are State-licensed. Once you complete your insurance E-ZQuote, MACH will provide that information to Massachusetts-licensed health insurance agents, brokers or carriers (hereinafter referred to as MACH “members.”) These members, in turn, may try to contact you directly, by phone, e-mail or by parcel post/U.S. Mail to provide you with your quotes. The site also provides information to consumers which may be helpful to them when making decisions about insurance.
Role of Massachusetts Consumer Health (MACH): MACH is not an agent, broker or carrier, nor does it provide quotes to the consumer. All quotes are provided by MACH members -- Massachusetts-licensed insurance agents that have agreed to the MACH terms of membership. MACH does not issue insurance contracts nor does it bind coverage. MACH does not recommend or endorse any carriers, companies or insurance policies. MACH is not responsible in any way for the conduct of any agent, broker or carrier, nor does MACH guarantee that any insurance agent, broker and/or carrier to whom it provides your insurance quote request form will either contact you and/or agree to provide you with coverage. MACH reserves the right to change, modify, remove or suspend any aspect of the site without notice or penalty.
Copyrighted Aspects of the Site: All services, materials and logos located on the site are owned by and property of MACH unless otherwise indicated. All materials are protected by United States copyright, trademark and (all) other applicable laws. Reproduction or use, by any means, of any materials, service marks, logos, other site content, or the name “Massachusetts Consumer Health” and its acronym. “MACH”, without the prior written consent of MACH, is expressly prohibited by law.
Consumer information: All consumer information that is submitted or collected by using the site or other online forms on the site, by e-mail, or in any other manner, are protected per the terms of the site's privacy policy. Consumers are assumed to have read and understood the site’s Statement of Privacy and Security before using the site.
Information about Children: We do not knowingly collect online contact information or personally identifiable offline contact information about children from children. We do not entice children by the prospect of a special game, prize or activity, to divulge any information about themselves for any reason. We do not knowingly accept or store personally identifiable information submitted by children, nor do we respond to, promote to, market to, or communicate with children known to be younger than eighteen years old. Information – Why it is Collected and how it is used: When you complete a health insurance E-ZQuote online and submit that information through the site it may be forwarded to our members, who may use it in a variety of ways to determine and/or offer the best rates to you. Health Information: All Massachusetts-licensed individual health insurance plans require you to submit certain personal information concerning your medical history in order to process your request. This may include, but is not limited to, your height, weight, medical history (for diagnoses of heart condition, anemia, headaches, diabetes, cancer, and other health disorders) and current health insurance status. Consequently, we cannot facilitate your health insurance quote request with out this information. By submitting this information you explicitly agree to the disclosure of this information for the purpose of obtaining a health insurance quote from a MACH member. We do not maintain records of your personal health information at MACH, nor do we make any other use of the information other than to pass it along to the licensed agent or insurer who will provide the quote/policy to you.
Dispensation Agreement: By submitting your personal information, you fully understand and consent to the transfer.
